At Amerigroup, we meet providers where they are--from small solo practices to large, fully-integrated health systems. We partner to find the payment model, data tools and care management support that works best for each provider's unique situation.

From our local Provider Relations representatives to our national Provider Services team, we're committed to providing one-on-one support to not only simplify the administrative component of patient care but also collaborate to meet the challenges of an evolving health care landscape.

Offering financial incentives that encourage quality care

We're a leader in collaborating with our provider communities to create quality incentive programs that work—providing better health outcomes for members, enhanced capabilities and financial performance for providers, and better overall results for our state partners. Amerigroup goes beyond just offering financial incentives and actually works alongside providers to help achieve improved outcomes through –

  • Data analytics information, including members currently in the hospital, recent emergency room visitors, and members with gaps in care
  • Onsite support to outreach to members who are overdue for a physician visit
  • Quality coaches who educate and support each practice to build systems for measuring and improving their quality measures as part of an ongoing quality improvement program

Supporting safety net providers

Amerigroup health plans contract with all available and willing Federally Qualified Health Centers (FQHC), Rural Health Centers (RHC), county and public health departments, community mental health centers, community service boards, and critical access hospitals. We have a strong collaborative relationship with the National Association for Community Health Centers (NACHC) and the primary health care and rural health care associations. Across our affiliate plans, more than 130,000 members are assigned to FQHCs, with approximately 44 percent of that membership in an FQHC participating in one or more of our shared savings/risk and incentive programs.

Helping providers build medical homes

Amerigroup believes in the power of patient-centered medical homes (PCMH).

  • We employ PCMH medical practice consultants who outreach to interested providers and help them apply for and obtain PCMH recognition.
  • We help practices with onsite support of care coordination activities.
  • We have quality coaches that help each practice build systems for measuring and improving its quality measures as part of a continual quality improvement program.
  • We provide enhanced payment to PCMH practices based on their level of NCQA recognition or Joint Commission certification.

Building Patient-Centered Medical Homes

Amerigroup works with hundreds of physicians in our states to create patient-centered medical homes that help doctors improve access and quality of care for their patients.
