Our Dual Medicare Advantage Special Needs Plans
Chronic Conditions Special Needs Plan (C-SNP)
Amerigroup has C-SNPs for people who have diabetes, a cardiovascular disorder or chronic heart failure. If you have one of these conditions and live in an area we cover, you can enroll in a C-SNP for coverage designed specifically for your needs.
Plans vary depending on where you live. You can use our search tool to see which plans are available to you.
Dual-eligible Special Needs Plan (D-SNP)
You can enroll in a D-SNP if you are both:
- Eligible for Medicare Part A and Part B, and
- Eligible for medical assistance under a State Medicaid program
To be eligible for Medicare, you need to be 65 or older or have certain health conditions. Medicaid is based on income levels instead of age. If your income qualifies for assistance, you can be younger than 65.

Institutional Special Needs Plan (I-SNP)
You can enroll in an I-SNP if you both:
- Have Medicare Part A and Part B, and
- Live in one of our network nursing homes or assisted living communities (or other plan-approved locations)
Medicare Advantage HMO Plans
If you don’t qualify for a SNP, our Medicare Advantage HMO plans still have many benefits. They cover things Original Medicare doesn’t cover, such as prescription drugs, dental, and vision.
