Making the change to value-based care:
In Texas, we already exceed the State’s targets for 2021 for Provider participation in quality-based alternative models (APMs) by 14%.
Integrating physical and behavioral health and social supports to improve outcomes:
Our Georgia plan lowered the number of prescriptions filled per 1,000 children in foster care by 3% for all psychotropic medications and by 8% for antipsychotics.
Reducing gaps in care:
Through our child checkup reminder program for well-child visits, screenings, and immunizations, we have closed gaps in care for members by:
- 52% in our Georgia plan
- 50% in our Florida plan
- 42% in our Iowa plan
